What's the point of ED consultants? Whilst all EDs are consultant led there is no doubt that for the most part they are not consultant based in that senior doctors are rarely present overnight and during all ‘out of hours’ periods. Perhaps ‘out of hours’ is an anachronism for our specialty as the patients still come, and still need help. Traditionally the night and late shifts are not led by consultants, but are there really any advantages to basing a service with consultants on the shop floor overnight? Perhaps it makes little difference? Aruni Sen and colleagues in Wrexham have a service delivery model that does put consultants on overnight and they have clearly shown the advantages of senior presence in the department (see page 366). This is really important data that we can use to argue for additional expansion of consultant numbers and I urge you...

The Quarter is the name given to a report released by the NHS under the name of the deputy chief executive David Florey. It provides a ‘definitive account of how the NHS is performing at national level against the requirements and indicators set out in the NHS Operating Framework 2011/12.1 The December release covers the July–September 2011 (Q2) quarter, the second period in the 2011/2012 performance year’.1 The report says that ‘in spite of the significant challenge of beginning to deliver on local quality, innovation, productivity and prevention (QIPP) plans, standards of safety and quality have been maintained or improved’. It's dry, dreary and turgid stuff to read. It is 64 pages long and covers many facets of the NHS. Introduction Major trauma remains the leading cause of death among children over the age of 1 year,1 with 47% of all non-natural deaths in children between 28 days and 18 years of age occurring as a result of road traffic accidents.2 The unintentional injury death rate in children has declined since the 1980s.3 Despite this there are 1120 deaths from unintentional injury among 0–19-year-olds in England per annum (rate of 8.6 per 100 000 population).4 Compared with the adult population, the absolute number of cases of major trauma in children is small, which has implications for how their trauma care should best be structured. Data are incomplete, but this may account for as few as 300 cases per year in the UK.5 Currently, children suffering major trauma are delivered from the scene of injury to the nearest emergency department and subsequently transferred to...

No Such Thing as “Just” a Nail Bed Injury
Fairbairn, NeilAbstract: Nail bed injuries are the commonest pediatric hand injuries presenting to the emergency department. If managed correctly, the patient recovers quickly and complications are rare. However, failure to appreciate the complex anatomy of the perionychium and the importance of exploration, washout, and repair can result in devastating complications. These injuries are often underestimated and, consequently, delegated to the most junior and inexperienced staff. The case of a 6-year-old boy with a flexor sheath infection and devitalized fingertip 1 week after injury is presented. Emphasis is placed on the soft tissue and underlying bony injury that was overlooked at initial presentation, and how this led to serious infection requiring amputation. Thorough assessment of these common injuries and a high index of suspicion are essential.
